What is a Consumer Directed Services (CDS)?

A Consumer Directed Services (CDS) is a Medicaid based program designed to assist people with disabilities with receiving the attendant services needed to remain in their homes or a home setting of their choice. Participants in the CDS program are trained to hire attendants to assist in daily task like meal preparation, cleaning, etc. Golden Years Health Care Services LLC's CDS program is a consumer controlled program which means the person who receives CDS is the employer of the attendant. The program participant will be responsible for locating, hiring, setting work schedules, and managing attendants.

What types of services can I recieve through CDS?

Qualified participants may receive some or all of the following services:
  • Bathing
  • Grooming
  • Dressing
  • Transferring
  • Toileting
  • Ostomy/Colostomy Hygiene
  • Obtaining medication
  • Range of motion
  • Meal preparation/feeding/cleanup
  • Housekeeping/Laundry
  • Errands/Transportation
  • Medical transportation

What is a Service Plan?

A service plan is a written document that specifies the services and support that are to be completed by the attendant to meet the preferences, choices, abilities and needs of the CDS participant. The CDS participant assist the attentant in directing those services and supports.

What are the qualifications to receive CDS?

  • Be eligible for MO HealthNet  (Medicaid) .
  • Be 18 years of age or older.
  • Be able to direct his or her own care.
  • Be physically disabled.
  • Be capable of living independently with the services of a personal care attendant.
  • Require at least a nursing facility level of care.

How can I become a CDS participant?

Once you meet the minimum qualification to receive (CDS), click on             Contact US  to input you personal information or call us at 314-566-9120 and someone will assistance you in completing a preliminary screening as well as referring you to Missouri Department of Health and Senior Services (DHSS) . We will also help you get your personal care attendant register and prepared for employment.

Who can become a Personal Care Attendant?

Any qualified caregiver chosen by the CDS participant including a family member other than a spouse or anyone under the age of 18 can become a Personal Care Attendant.

A CDS program participant is the legal employer of his or her personal care attendant. Participants are responsible for interviewing, hiring, scheduling, directing and, if deemed necessary, firing an attendant. Personal care attendants do not work for Golden Years Health Care Services LLC.

Attendants must visit Golden Years Health Care Services LLC and bring two forms of ID, (Social Security Card and Drive's License). There is also a $14.25 onetime FCSR registration fee which is waved if hired.

All Personal Care Attendants must complete and pass a background through Family Care Safety Registry (FCSR), Employee Disqualification List (EDL) , E-Verify, and starting in 01/01/2017 a criminal background check through Missouri Automated Criminal History System (MACHS) . All background checks will be completed by Golden Years Health Care Services LLC.

The Personal Care Attendant cannot begin working for the CDS Participant until the participant assessment has been completed and approved to receive services by DHSS.

Where do the money come from to pay the Personal Care Attendant?

All CDS programs are funded by MO HealthNET (Medicaid) and participants who receive Medicare or have private insurance will not be eligible for the program. All participants must be eligible to receive MO HealthNet (Medicaid) in order for a Personal Care Attendant to be paid.

Once services are verified, approved, and  billed to MOHealthNet in accordance with the care plan authorized by DHSS, the attendant is paid by money received from a participant's MO HealthNet (medicaid) account for authorized care plan services provided to the participant.
